Form distributions, tracking progress and retracting forms

How to monitor and track form completion, downloading complete forms and retracting forms that are stuck!

When a form has been 'distributed' to a group of clinicians, admins can monitor it's progress to ensure completion, download completed forms and flag forms that aren't progressing.

  1. Searching: Find individuals based on their name.

  2. Filtering: Only show forms based on their 'status, section completed, and if it's progressing.

  3. Form status: Not started, Incomplete, Completed.

  4. Form progress: Indicates what stage of completion the form is at.

  5. Form actions: A detailed view of form completion, download complete PDF form, copy form URL.

Section Progress

When smart reminders are enabled, there will be additional columns that display on the forms response list view; Section progress and number of reminders sent.

What does each status mean?

  • "On Track" - if a reminder has not been sent yet to the current section assignee. Note: If a section does not have a reminder set then the status will display as on track

  • "Reminded" - a reminder has been sent to the current section assignee

  • "Not Progressing" - the section has been flagged as not progressing

Form Actions

Clicking view in line with a clinician name will open a new dialogue to display detailed form information.

  • Form progress: a detailed view of progress

  • Form preview: see a live view of form progress & download completed PDF

  • Actions: Retracting a form to previous section, reset form to start, reassign form and remove form (more information below)

Form Progress

  • The current section assigned and section progress status

  • who the section is assigned to, when it was assigned and the type of notification

  • The number of reminders that have been sent

  • The next scheduled reminder (if there is a scheduled reminder)

  • Previous scheduled reminder (if a previous reminder was sent)

Form Preview

Changing the view from form progress to form preview will show the form progress:

  • Incomplete forms: Preview the live data

  • Completed forms: Preview and download a PDF


Form Actions: Retracting & Reassigning forms

Form actions allow an admin to retract forms. After clicking 'view' in line with a clinicians name, press the 'actions' button to provide options for retracting a form.

Retract to previous section:

Retracts the form to the previous section of the form, allowing changes to be made or a new person to be assigned to the form (🚨 Retracting it back to a previous step will lose any draft data saved in the most recent state).

  • For Example: an assessor has started their form, saved it as a draft, then a dashboard admin retracts it back to the 'clinician' stage. All draft data at the assessor stage will be lost. Any data originally entered by the clinician at their stage will still exist in their form - it just retacts it back to the 'ready to submit' stage).

Reset form and start again:

Completely restarts the form from the beginning of the form (🚨 All data already entered into form will be lost by selecting this option).

Reassign Form:

A dashboard admin can go to a form assigned to a clinician and then choose to reassign it to someone else (e.g. a new assessor). For example if the person has assigned it to an assessor who is on leave or who has left then a dashboard admin can see this in the dashboard and reassign straight away. The end user can still retract and reassign from their app, but this functionality allows the dashboard teams more functionality to support clinicians using forms.

Remove Form (from person):

If you accidentally assigned a form to someone, they have left your organisation or aren't required to complete a form you can remove them from a distribution directly from the dashboard as one of the form actions.

You can also reinstate the form if you accidentally remove it. Removing a form for someone will mean that it no longer shows up for them in the app and they won't have access to it.

Use the filter options in the form distribution list to see any removed forms.
