How do clinicians complete ad-hoc forms

Clinicians can submit a range of forms such as recall, unrostered overtime claims direct to your hospital for approval

Clinicians can now submit Ad-Hoc ('on demand') forms from Med App. This lets you start, save and submit forms directly to your hospital, assessor or supervisor for approval, saving time and paper!

Steps to start and complete a form

NOTE: Information about submitting on-call and recall to duty forms

Please use this form if you are on-call and recalled to duty, to complete either:

Remote work by electronic means from off-site i.e. telephone and/or computer

Physical return to the health service

Please ensure the following:

  1. Select whether you remotely worked electronically or whether you physically went into the health service

  2. Record the reason for recall, the start and end time of the recall, and enter travel details if applicable

  3. Ask your Head of Unit or supervising consultant to approve. Contact Medical Workforce Unit if you need support

  4. Must be submitted to the Medical Workforce Unit by 0900 Monday of the pay period end

What if you don't see 'My Forms'

If My Forms doesn't appear in your Med App:

  1. Check your app has the latest version 4.14.12. Go to the App Store or Google Play

  2. Your hospital may not have this functionality set up. Click the chat below to find out how to enable this feature.
