Step 1: Gather the Required Information

Before reaching out to us, please ensure you have the following details of the clinician you wish to add:

Basic Information:

  • Title: The professional title of the clinician (e.g., Dr., Mr., Ms.).

  • First Name: The given name of the clinician.

  • Last Name: The surname or family name of the clinician.

  • Role: The specific role or position of the clinician within your organization.

Address:

Street 1: The primary address line.

  • Street 2: Any additional address details, if applicable.

  • City: The city where the clinician is located.

  • State: The state or province of the clinician's address.

  • Zip: The postal or ZIP code.

Contact Info:

  • Email: The clinician's professional email address.

  • Phone Number: The contact number for the clinician.
