To initiate the reimbursement process, you'll need to complete the "myHSA Travel Form" (attached with this article) and attach it with your claim.


3. Documentation Required

Now, it's essential to provide proper documentation to support your claim. The adjudicators will need to see the following included in your claim:

  1. Completed Travel Claim Form: Ensure the form is accurately filled out with all required details included.

  2. Proof of Distance: Provide a map or equivalent documentation showing the distance traveled (home to the medical center). A standard tool like Google Maps will suffice.

  3. Necessity Proof: Include documentation or proof that the medical service could not be accessed closer to your residence. Medical documentation or a doctor's letter stating such is sufficient.

  4. Appointment Confirmation: Submit evidence that your appointment took place, such as an official service receipt, signed invoice, or email from the medical provider.

  5. Doctor’s Note for Companion Travel: If a travel companion was required for medical reasons, include a note from your doctor.

4. Claiming Additional Expenses

Beyond the core medical expenses, you can claim additional costs such as accommodations, mileage, parking fees, and meals. It's more than just gas money, and it can significantly ease the financial burden of the trip.

To calculate meal expenses, you can use the "simplified method" at $23 per meal per person or submit individual receipts. Remember to also retain receipts for costs like parking and accommodation fees.

The mileage rates aren't uniform across the board; they vary by province. To get precise information on the rates applicable in your province, you can check the mileage in the form attached below.
