A blood pressure monitor may be eligible under your plan if you have the category "Durable Equipment" and sub category ""Any equipment or device, deemed medically necessary and specifically prescribed for the treatment of a medical condition." sub category on your list of covered items.

We require the following criteria:

1. The equipment must be prescribed by a healthcare professional. The prescription must confirm the medical necessity of the equipment. It must state clearly that it is medically necessary (in those words), not just recommended, suggested, or beneficial.

2. The prescription must indicate the specific medical condition the equipment is intended to treat.

3. The equipment item must be specifically named in the prescription.

4. The equipment you purchased must match the prescribed item.

5. The prescription must state the practitioner's name and credentials, the patient's name, and the date it was written.

By meeting these requirements, the claim is considered eligible. If these conditions are not met, the claim will be denied until the above conditions are presented.

If the above criteria is met, it can be submitted under: "Any equipment or device, deemed medically necessary and specifically prescribed for the treatment of a medical condition."
