Customizing your LocalVisits Form

Lets dive into building the best customer form flow for your business!

This article will take you through how to start editing your Form flow for LocalVisits. Need some further information on best practices? Click the button below to take a look before starting to build.

Ready to get started?

  1. Go to your 'Settings' at the bottom right of your screen

  2. Click into 'LocalVisits' โ†’ 'Check-in' โ†’ 'Forms'

This will allow you to view the form editor (see below) where you can customize your questionnaire.

Starting with the 'Build' tab

Format your LocalVisits form before applying rules which will flag your clients. Let's get started in the 'Build' tab.

Welcome Page

  1. The welcome page is the first screen your customers will see when they begin the form

  2. Click the 'Pencil' icon on the left of the field to edit your logo and welcome text

Tip: Your logo should be jpg or png format with a white background to make sure its clear when the form is sent!

Add a Question

Following your welcome page, you can choose to include additional questions, please refer to the best practices above when choosing your questions.

Select 'Add a Question' to begin.

There are two types of questions that can be added:

For both question types you can:

  1. Customize the question and make the question 'Required' by your customers

  2. Choose to display this information on your LocalVisits dashboard

Text Based Questions:

  • Can include an 'Name', 'Email', 'Phone Number' or general 'Text' type for your customers to enter text.

Multiple Choice Questions:

  • Here you can input your question as needed

  • Offer multiple pre-decided answers within your form and make these multi or single select

You can also reorder your questions as needed by using the arrow icon to drag and drop in the best order for your customers form flow.

Exit Registration Page

This page should indicate next steps to your customer!

It's great to let them know here that you will be in touch shortly so they know when they can come in for their appointment.

Alternatively, if you will not be able to see them that day, indicate you will get in touch to help them with further information.

Add Flags to your Questions

At the top of the page, navigate back to the 'General' tab. Once the questionnaire is built out we can now add flagging logic.

Adding this logic for the form flow will allow you to flag clients checking in that you may need to reach out too for further information, or reschedule their appointment.

Click 'Add a Rule' to get started

  1. Select a question from the survey we built that you would like to flag an answer from i.e. "Have you travelled in the last 14 days?"

  2. You can then edit the 'Condition' to: 'Equals' or 'Does not Equal' i.e. 'Equals

  3. The 'Value' will reflect the information you would like to flag and will give you a list of answers from the form we built, i.e. 'Yes'

Depending on this, your customers will be automatically flagged within the LocalVisits dashboard for you to easily see who you may need to follow up with via the LocalMessages integration!

Your LocalVisits form is now customized and ready to send!
