Photos on Failed Task completion allow your organization to be able to enable or require photos to be uploaded by drivers when they have to fail a task in the Onfleet driver app.
Your organization can now capture photos of failed deliveries the same way you do for successful ones, so you have clear, documented evidence for every stop.
Common scenarios where this helps:
Gated or locked entrances: Photo proves the building was inaccessible.
Construction or road closures: Visual evidence backs up why the driver couldn't reach the address.
Damaged goods: Photo documents the condition before return.
High-value or regulated shipments: Pharma, alcohol, and age-restricted goods all benefit from consistent proof.
Cleaner billing and redelivery decisions: Your ops team can see the blocker and decide whether to retry, reschedule, or adjust charges without a back-and-forth with the driver.
How to Enable Photos on Failed Task Completion
To enable the photos on failed task completion do the follwoing:
Log into your Onfleet dashboard.
Go to Settings located on the bottom left corner
From Settings select Driver App
Under Completion Settings, you'll now see two sections: On Success and On Failure.
5. Choose your failure photo mode
Select one of three options for On Failure:
Disable – Drivers don't need to attach a photo when a delivery fails. (This is the current behavior.)
Enable – A photo is optional. Drivers can mark the delivery as failed with or without uploading a photo.
Require – A photo is mandatory. The driver cannot mark the delivery as failed until they attach one.
How drivers experience it
When a driver marks a task as failed in the mobile app, they will either:
See a prompt to add a photo (if you've enabled optional photos)
Be required to attach a photo to proceed (if you've set it to required)
Once attached, the photo is locked to that failed completion and appears in the task's history alongside the failure reason.

Failed-completion photos appear in the same place as successful-completion photos:
In the task's details and history view in the dashboard
In CSV exports (the photo link is included)
Accessible by admins, dispatchers, ops teams, and support staff
