Evaluation templates can be created and then applied to multiple seasons within your organization. Once applied, the template can be further modified to meet specific seasonal needs. When form templates are applied from season to season, any changes made to the form will only impact the season the changes are made in, and not the master template itself. Follow the steps below to begin creating your player evaluation form template:
1. Creating Player Evaluation Form Templates
1. From the navigation, choose Settings, then select Forms & Docs > Web Forms. Next, click on the mini menu next to the Template dropdown and select ‘Add New Template’.
2. Enter a name for the template and select Evaluations from the Form Type dropdown menu. Click ‘Add' once complete.
3. Using the mini-menu, begin to build out the form by adding form items by dragging and dropping them onto the template. Evaluation-specific form items can be found by choosing the Free-form Text option. Once selected, use the Data Mapping dropdown menu to add items to the template.
Creating Player Evaluation Forms Per Season
In order to allow flexibility and customization, player evaluation forms can also be created per season. Follow the steps below to create a seasonal player evaluation form:
1. From the navigation, select Settings > Teams > Seasonal Forms. Select the program and season, and then click on the mini menu and select ‘Add Seasonal Form’.
2. Enter a name for the seasonal evaluation form. Next, choose who Can Submit the evaluation - Full Admins (selected by default), Directors, Players, and Relatives, or Team Staff, and who Can View the evaluations - Full Admins (selected by default), Directors, Players, and Relatives, or Team Staff. Once complete, click ‘Add’.
Note: Full Admins will be selected by default for both options.
3. Using the mini menu, begin to build out the form by adding form items by dragging and dropping them onto the template. Evaluation-specific form items can be found by choosing the Free-form Text option. Once selected, use the Data Mapping dropdown menu to add items to the template.
2. Applying an Evaluation Form Template to a Season
Use the steps below to apply an existing evaluation form template, created in the Forms product, to a season:
1. From the navigation, select Settings > Teams > Seasonal Forms. Select the program and season, and then click on the mini menu and select ‘Add Seasonal Form’.
2. Enter a name for the seasonal evaluation form. Next, choose who Can Submit the evaluation - Full Admins, Directors, Players, and Relatives, or Team Staff, and who Can View the evaluations - Full Admin, Directors, Players, and Relatives, or Team Staff. Once complete, click ‘Add’.
Note: Full Admins will be selected by default for both options.
3. To apply an existing template, click on ‘Add Section’, then select ‘Templated Sections & Items’ and use the dropdown menu to select your template. Click ‘Add’ once complete.
4. Once a template has been applied, it can be modified as needed to accommodate any seasonal needs.
Use the mini menu to add new form items to a section, edit the section title, or delete the section.
Evaluation-specific form items can be found the choosing the Free-form text option.
Layouts can be modified by dragging and dropping form items on the seasonal evaluation.
Individual form items can be edited by hovering over the item and clicking on the ‘Edit this item’ option.
Note: Any changes made to a seasonal form template will not affect the master template or any other seasons where the template is also being used.
Admin Note: Once submissions have been made against an evaluation form, the name and permissions may still be edited, but form type and role values will be locked.
3. Submitting Seasonal Player Evaluation Forms
Users who have been granted permission to submit evaluations will navigate to the Team’s Page and follow the steps below:
1. Once on the Team Page, scroll to the Rostered Players section. Click on the Form icon (paperclip) aside from the player’s name. Select the form title from the dialog box, and the seasonal evaluation form will then display.
2. Fill out all information as needed and click Save once complete.
4. Viewing Player Evaluations
Via Player Profile
1. Navigate to a player’s profile and click on Player. Select the evaluation you wish to view using the Form dropdown.
2. Click on the Report button to view the evaluation. If a player has multiple evaluations that have been submitted, there will be a line item for each submission.
Via Team Page
1. Navigate to the Team Page and click on the Reports. On the Reports view, use the Report dropdown menu and select Evaluations.
2. Once Evaluations has been selected from the menu, choose the evaluation you wish to view from the Form dropdown.
3. Evaluations will be listed on the screen. If a player has multiple evaluations that have been submitted, there will be a line item for each submission. Click on the Report button to view the evaluation. Full admins will also have the option to modify evaluations by clicking on the Edit icon or remove an evaluation by clicking on the Delete icon.
Tip: Use the mini menu and select Configure Table Columns to modify the columns on the report to create a more customized display of the report. It can then be exported as a .CSV file, a PDF, or printed.
