Users who have been granted permission to submit evaluations will navigate to the Team’s Page and follow the steps below:
1. Once on the Team Page, scroll to the Rostered Players section. Click on the Form icon (paperclip) beside the player’s name. Select the form title from the dialog box, and the seasonal evaluation form will then display.
2. Fill out all information as needed and click Save once complete.
