Creating ID Card Templates

Formatting the ID Card Size

The data attributes used to build out ID card templates are unique to templates created under the ID cards category. ID cards cannot be created outside of this category. Follow the directions outlined below to begin building out an ID card template. If you need additional assistance creating your ID Card templates, please contact the support team.

1. From the navigation, select Settings > Forms & Docs > Printable Docs. Under the Form Groupings section, click ID Cards and select the Create Form button to begin creating your new ID card.

  • Note: Click on the name of the ID card or the edit icon (gear icon) to view/edit an existing ID card template.

2. Use the form editor to begin creating the ID card. Begin by entering a Form Name.

3. Important: If you are a league or association, select the Club Registration Data category from the ‘Print forms based on’ menu, and if you are a club, select the Participants data category. Choosing the correct category for your organization will properly map the data fields for the ID cards.

4. When starting a new ID card template, the page size defaults to the standard 8.5 x 11 without any margins or columns on the blank form. Click on Edit Layout to open the Printable Form Settings options and set your size for the ID cards.


5. Use the column, row, page, and margin configurations to set the size of your ID cards. The example below shows 2 columns with 5 rows per page (making 10 cards per page). However, the column, row, and margin settings can all be modified depending on the size of the ID card your organization needs.

6. Once you have selected your ID card form settings, click Save Settings. Upon saving, the page will reformat to display your ID card formatting. Use the top left-hand corner card with the grid display to build your template.

7. To begin adding basic form items to the template, select Add Form Items. Click on a form item button for it to appear on the template page. Hover over the form item to see the edit icons (drag, edit, and remove). Below is an overview of each basic form item type:

  • Note: The form does not automatically save any changes, so remember to click Quick Save often.

  • Label: A label can be used as an open text attribute to create or include custom labels or information.

  • Print Date: Will generate a datestamp of when the document is accessed

  • Image: Option to add an organizational logo, watermark, etc. Image size, opacity, and other properties can be modified as necessary.

  • Data Field: Most commonly used to pull in singular data attributes for team or player information, such as seasonal team name, player name, or player DOB.

  • Data Table: Most commonly used to pull in a series of data attributes, such as home team staff and/or players and away team staff and/or players.

  • E-Signature: Will prompt for the user to provide an e-signature (not commonly used for ID Cards)

  • E-Initals: Will prompt the user to provide e-initals (not commonly used for ID Cards)

  • Container: Option to create a visual divider or a comments box for the template.

Adding Player, Team, or Season Information with Data Fields

1. To add player, team, or seasonal information to the ID card, select Add Form Item > Data Field > hover over the data field > click the edit icon.

  • Note: When adding data fields onto the template, they will appear as ‘- -none- -’ until edited.

2. Click the Select icon to choose a data field to link to the input. A menu will appear on the screen with a list of options to select. Common selections might include:

  • Player Information: Club Player First & Last Name, Club Player DOB, Team Player Status, and Club Player UUID.

  • Team Information: Seasonal Team Name, Seasonal Team Age Group, and Seasonal Team Status.

  • Season Information: Season Name, Season Team Type, Seasonal Team Gender, Full Season Group Name

3. Use the available formatting options to customize the data field. Font type, font size, and color can all be modified depending on template needs.

4. Once done editing, click the Edit Item tab to close out of the edit options dialog box. Use the drag icon (yellow arrow) to move the item on the template.

  • Important: The form does not automatically save any changes, so remember to click Quick Save often.

Using Labels

Labels can be used as open-ended text boxes to add descriptions or instructions to the template, or can also be used to describe adjacent data fields on the template.

1. To add labels to the ID card, select Add Form Item > Label > hover over the label field > click the edit icon.

  • Note: The label will read ‘The Label’ on the template until edited.

2. Enter the text to appear for the label. Use the available formatting options to customize the data field. Font type, font size, and color can all be modified depending on template needs.

3. Once done editing the table, click the Edit Item tab to close out of the edit options dialog box. Use the drag icon (yellow arrow) to move the table on the template.

  • Note: Be sure to click Quick Save often. The form will not automatically save as you are creating your ID card.

Adding Team Member Photos

Profile photos uploaded onto team member profiles can be added to ID cards so season officials can easily identify and verify player and staff members.

1. To add a player photo to the ID card, select Add Form Item > Image > hover over the image field> click the edit icon.

2. For the image type, choose Dynamic Image, then click the Select button and choose Club Player Photo, Club Team Staff Photo, or Participant Photo from the Database Field list.

3. Use the available formatting options to customize the image on the template. Image width x height, opacity, and border color can all be modified depending on template needs.

4. Once done editing the table, click the Edit Item tab to close out of the edit options dialog box. Use the drag icon (yellow arrow) to move the table on the template.

  • Note: Be sure to click Quick Save often. The form will not automatically save as you are creating your ID card.

Adding Images

Adding images provides options to add an organizational logo, watermark, or picture to the ID card template. Image size, opacity, and other properties can be modified as necessary.

1. To add an image to the ID card, select Add Form Item > Image > hover over the image field> click the edit icon.

2. For the image type, choose Static Image, then click the Upload/Find Image button to locate the image on your device or paste the image URL into the text box.

3. Use the available formatting options to customize the image on the template. Image width x height, opacity, and border color can all be modified depending on template needs.

4. Once done editing the table, click the Edit Item tab to close out of the edit options dialog box. Use the drag icon (yellow arrow) to move the table on the template.

  • Note: Be sure to click Quick Save often. The form will not automatically save as you are creating your ID card.
