The data attributes to build out game card templates are unique to templates created under the Game Cards category. Game cards cannot be created outside of this category. Follow the directions outlined below to begin building out a game card template:
From the navigation, select Settings > Forms & Docs > Printable Docs.
Under the Form Groupings section, click Game Cards and select Create Form to begin creating your new game card.
Note: Click on the name of the game card or the edit icon to view/edit an existing game card template.
3. Use the form editor to begin creating the game card. Begin by entering a Form Name.
Important: The form does not automatically save any changes, so remember to click Save Changes often.
4. To begin adding basic form items to the template, select Add Form Items. Click on a form item button for your selection to appear on the template page. Hover over the form item to see the edit icons (move, edit, and remove). Below is an overview of each basic form item type:
Label: A label can be used as an open text attribute to create or include custom labels or information.
Print Date: Will generate a date stamp of when the document is accessed/printed
Image: Option to add an organizational logo, watermark, etc. Image size, opacity, and other properties can be modified as necessary.
Data Field: Most commonly used to pull in singular data attributes for team or game information such as team type, game location name, or game number.
Data Table: Most commonly used to pull in a series of data attributes such as home team staff and/or players and away team staff and/or players.
E-signature: Used to gather an electronic signature from the user for a waiver/form.
E- initials: Used to gather electronic initials from the user for a waiver/form.
Container: Option to create a visual divider or a write-in comment box for the template.
Adding Staff/Volunteer Roster Information
To add staff/volunteer members or participant rosters to the game card, select Add Form Item > Data Table > hover over the table > click the edit icon.
The edit options for the data table will appear on the screen. From the Data Source drop-down menu select ‘Home Staff’, ‘Home Players’, ‘Away Staff’, or ‘Away Players’.
Click Add Columns to add table columns that will pull in attributes associated with your data source selection. Information such as staff names, player names, roles, or jersey numbers can be added.
Note: You can also select ‘Add Blank Column Rows’ to add blank column headers to be used for write-in information that needs to be recorded on game cards. Attendance, goals, or misconduct are just a few examples of information that can differ from game to game and would need to be written in for each staff member or participant.
To change the label of a table column, hover over the label and select the Edit Column Label icon. A dialog box will appear on the screen prompting you to change the text. Enter the new label and click Save.
Use the available formatting options to customize the table. Font type, font size, header options, row count, and row width can all be modified depending on template needs.
Once done editing the table, click the Edit Item tab to close out of the edit options dialog box. Use the move icon to move the table on the template.
Important: The form does not automatically save any changes, so remember to click Quick Save often.
Adding Game Day Information
To add game day information to the game card select Add Form Item > Data Field > hover over the data field > click the edit icon.
Note: When adding data fields onto the template, they will appear as ‘- -none- -’ until edited.
Click the Select icon to choose a data field to link to the input. A menu will appear on the screen with a list of options to select. Common selections might include:
Game Information: Game date, game ID, game location name, game surface name, game competition name, game full season group name,
Home Team Information: Home team name, home team type, home team age group, home team Universal ID
Away Team Information: Away team name, away team type, away team age group, away team Universal ID
Use the available formatting options to customize the data field. Font type, font size, and color can all be modified depending on template needs.
Once done editing the table, click the Edit Item tab to close out of the edit options dialog box. Use the drag icon (yellow arrow) to move the table on the template.
Important: The form does not automatically save any changes, so remember to click Quick Save often.
Using Labels
Labels can be used as open-ended text boxes to add descriptions or instructions to the template, or can also be used to describe adjacent data fields on the template.
To add labels to the game card, select Add Form Item >Label > hover over the label field > click the edit icon (blue pencil).
Note: The label will read ‘The Label’ on the template until edited.
Enter the text to appear for the label. Use the available formatting options to customize the data field. Font type, font size, and color can all be modified depending on template needs.
Once done editing the table, click the Edit Item tab to close out of the edit options dialog box. Use the drag icon (yellow arrow) to move the table on the template.

Adding Images
Adding images provides options to add an organizational logo, watermark, or picture to the game card template. Image size, opacity, and other properties can be modified as necessary.
To add an image to the game card, select Add Form Item > Image > hover over the image field> click the edit icon (blue pencil).
Click the Upload/Find Image button to locate the image on your device or paste the image URL into the text box.
Use the available formatting options to customize the image on the template. Image width x height, opacity, and border color can all be modified depending on template needs.
Once done editing the table, click the Edit Item tab to close out of the edit options dialog box. Use the drag icon (yellow arrow) to move the image on the template.
