Setting up Custom Registration Forms with the Form Builder

1. To begin customizing your registration form, first open the program and season that you would like to work in. Within the Programs, click on Participant/Player Form.

2. A grid area will appear on the screen. This expandable grid will adjust in size as you add registration questions to it.


Using the form builder options at the top of the screen, begin to build out your registration form. Options include:

  • Save Changes

  • Add Form Item

  • Insert Template

  • Save as Template

  • Public View

3. Options to select a pre-built template or build your own form are available. Starting every participant form from a template and modifying it to fit the specific season is recommended, but only needed if you need to collect additional information on top of the standard data fields below.

System Data Fields (all of these are system data fields collected via the public registration process):

Name - required

Parent info - required

DOB - required

Emergency contact info - optional

Gender - required

Photo - optional or required by the program

Primary phone - optional

Birth certificate - optional or
required by program

4. If you would like to use a template, click the green Add From Template button to reveal system templates (created by OTTO Sport) and the Custom Domain Templates (created by your organization).

5. To add one of these templates to your participant form, simply click on the name. Now you can begin modifying the form.

6. Every question on the form will have three buttons appear when clicked: move, edit, and delete. To move an item, click and hold the yellow button. Drag and drop the question to a new spot on the form.

7. Questions can be marked as a required response, set as conditional questions, and have fees tied to them.

Note: While it is an uncommon practice to include a participant form for a team invite program, if you need to collect additional information, forms can be used to do so.
