How do I Report on Form Responses?

If you have access to build reports in Participant Registration, you can report on all the fields/questions that were included on the participant form during the public registration process.

1. Navigate to the Reports > Custom Reports. Click Add New Report.

2. Click on New Blank Report.

3. Under the data categories, click Registration Data, then Participants.

4. Click Registration Info, navigate to the data field called "Form Responses" and click "+" to add to Report Data.

5. Select the season you are looking to report on. Note: Because each season has its own form, you can only report on form responses for one season at a time. To report on more than one season, create the report for the first season, then clone the report and select a different season to report on.

6. Add in any other data fields you'd like to see, such as participant name, registration data, household name, season group name, etc.

7. Give the report a title and save it

8. Run the report.

9. This will bring in ALL form questions that were on the form for that season, which may be a lot of data fields.

10. To narrow the data in your report to just a subset of the form responses (for example, if you only care about the jersey size or the player's USA Hockey number), use the "configure button" to check off which fields are of interest for this particular report.

11. Save the report again, which will save the configurations, so the next time you run this report, it will only show the fields you wish to see.
