What are Care Gaps?
Care gaps are a group of high yield regularly performed activities that help provide context to identify your patients needs. The goal of the feature is to quickly identify and track the different gaps for each patient. This helps your organization visualize where most of the care is being provided. Phamily can also provide your organization with care gap reporting on a quarterly basis.
Why is it beneficial?
Our program uses a feature to locate keywords in a conversation to help quickly identify and confirm care gaps. Identifying care gaps is an essential part of chronic care management as it utilizes our proactive care approach to monitor patients’ engagement. Care gaps help recognize which patients have missed their routine preventative services, by doing so, it will guide your clinic to: lead better health outcomes, early detection of diseases and provide maintenance for chronic conditions.
How it works in Phamily

This process is during normal workflow- when having a conversation with a patient:
When a message is sent/received, our system will process the text and suggest a care gap if one is relevant. These care gap suggestions will show up as gray tags (boxes) listed below.
Select: Appropriate tag
Clicking on the gray tag(box) will turn it purple, indicating that this care gap has been confirmed
Note section
If you are not on Stopwatch then care gaps can also be accessed through the notes icon.
Locate the patient's profile
Select: Add note
Press the correct note to be added or new note. The note section (tan box) will appear
Enter the note and approximate minutes
Select care gaps and choose from the drop down
Click Submit
Note: Only the items included in the drop down can be added. If there is a specific care gap that frequently comes to mind that you do not see listed, please contact us at support@phamily.com

Hover your mouse over the message, to the right of the text a small tag symbol will appear, click the tag symbol and a list of options will appear with a search bar as well.

You can add any additional information into the note section.

Care gap activity appears in the "recent activity" section of the patient's profile. You can also request a quarterly report from Phamily.

Yes, below the message tagged, it will say “user logged” then show the purple tagged care gap, if you hover your mouse over the purple care tag, to the very right of it a small “x” will appear, click the “x” and a pop-up message will appear asking if you want to delete the selected care gap, select yes.

No, you will be able to see all the activity you have done on your side through the conversation window, but a text does not get sent to the patient.
