To create a new form, navigate to ‘Contacts’ and select ‘Forms’ from the dropdown menu.

Once on the overview page for forms, select ‘New form’.

You can now choose a pre-set or start from scratch.

Following this, choose a public or private form. Private forms require a second step of verification.

Once on the overview page for the form, you can configure the design and attributes on the right hand menu. You can configure the design by selecting ‘Styling’ from the top right.

To configure content such as images, simply drag the blocks from the right side into your form. You can then select the image you’d like to use, as well as the alignment, dimensions, and corner rounding.

Beneath the content blocks on the right side, you can also drag and drop contact attributes such as names, birth dates, or emails. Email is always required for a sign-up form. Simply drag these into the form to include them.

To edit the attribute, click on ‘Edit’ to configure placeholder values, whether or not it is a default value, and more.

Once you have configured the main form, you can navigate to the ‘Success’ page of the form. Here, you can add an ‘Add to wallet’ button for a sign-up to allow customers to add their wallet pass to their mobile wallets. You can adjust the sizing and configuration of these buttons.

To determine the settings of a specific form, click on the form and you will be able to configure whether or not it can overwrite contacts, whether or not it can create contacts, if it requires approval, and whether or not it is active. Click ‘Submit’ after changing these to save any changes.

On this page, you can also find a link to the form as well as the QR code for the form. Simply click ‘Copy’ next to the form URL or ‘Download QR code’ respectively.
