Create a Credit Card Authorization Form with CVV code field.

In our Forms section, you can create a Credit Card Authorization form to send to your clients which includes the extra CVV field. You can start by choosing the Credit Card Authorization button under the Forms tab.

From there you will click the pink + Add New button.

Here you will create and name your form. You can customize the form by adding custom questions or paragraphs, for example, your Terms and Conditions. Simply click the + Add Question and/or + Add Paragraph/Acknowledge box and fill in your custom text.

Next, you'll see where you can include an option to authorize a specific dollar amount (for a specific trip). If this is not needed and you are creating a general form, click the No button.

FYI: We don't store the credit card security code because it's against PCI compliance rules. We use a security company to store all financial data securely and use tokenization to retrieve the card information to be read one time.

You have the option to add an E signature to any form. Just switch the button at the top of the E signature section from Off to On.

Once the client has submitted their information it will show up in the submitted forms section as well as in that client's Profile.

How to make your Credit Card Authorization Form your Default Form

1. Go to your Forms and select 'Credit Card Authorization Form'.

2. Copy-paste your Credit Card Authorization form link into the field at the bottom.

3. Click save and that will replace the link with yours.
