Go to Conversions and then Template Forms.
1. Forms
In this tab, you will find all your previously created forms.
By clicking on the template, the editing option allows you to:
Add additional fields
Modify the display order of your fields
Make a field mandatory or optional
You also have the option to "Add a form" if you wish to create a new one.
