Login to Time to Spare and click on "Outcome/Surveys" on the left hand side.
Click on "Create New Survey" in the top right corner
Make sure that "Grant Application" is selected from the dropdown in the top left corner:
You can then add questions and sections to the form. All the question types are on the right hand side. To add a new question, just drag and drop the question type from the right hand side into a position on the form.
To change the possible answers or question text, click on the little pen button and then the tick when done:
To add a cover page or a new section, use the options on the right hand side:
Once you are finished, press "Create this form" in the top right and give it a name.
