MDS Assessment Workflow Guide

Step-by-step guide for DOR's to locate, input initial and finalized MDS assessments in RESTORE Insights, even without final CMG.

Where to Find the MDS Assessment in the Platform

  1. Navigate to the Therapy Planner.

  2. Click the MDS Tab to select the MDS assessment you need to create.


Entering Initial MDS Discharge Assessment Details

  • Enter Not Applicable or use the Case Mix Estimator Report (Currently not available for all users) in the MDS Discharge Assessment for placeholder estimates.


Entering Discharge MDS Assessment

  1. Click Finalized to indicate that the assessment is ready.

  2. Enter the Finalized CMG into the MDS Discharge Assessment.

  3. Click Save Details.

  • Your changes should now be reflected in Section O and GG reports. If the MDS section of a report appears blank even after evaluations have been completed and signed, verify the following:

  • Finalize the MDS assessment in the Therapy Planning tab to ensure proper setup.

  • Double-check the Assessment Reference Date, as it needs to be correct for the report to generate.

  • Allow the system time to refresh after making updates. Typically, the data should update automatically within a short period. If the problem persists, revisit these points to ensure all requirements are fulfilled or try refreshing the system once more before escalating the issue.

Need to close the Episode of Care (EOC) before receiving the finalized CMG from the MDS coordinator? Follow these steps:

  1. Click Finalized.

  2. Select N/A for the CMG field.

  3. Close the Episode of Care.

    *After receiving finalized CMG from MDS Coordinator*

  4. Re-open the Episode of Care

  5. Enter Final CMG and Click Save Details

Note: To update the CMG Finalized Assessment in the Therapy Planner, the EOC must remain open. If you need to edit a closed Episode of Care (EOC) while another EOC is open, follow these steps:

  1. Temporarily close the currently open EOC.

  2. Re-open the previous (closed) EOC that you wish to edit.

  3. Add the required information, such as an MDS assessment, to the closed EOC.

  4. Close the previous EOC again after completing the necessary edits.

  5. Re-open the current EOC to continue care and documentation for the patient. This approach ensures that no overlapping modifications occur between active episodes.


Important Reminders

  • The Assessment Reference Date must be set correctly to ensure accurate data for Section O and Section GG. Additionally, always verify that the finalized MDS assessment reflects the correct Assessment Reference Date and confirm proper data synchronization. Allow the system some time to refresh if updates are not visible immediately.
