Using the Docs Due List

The Docs Due List helps therapists and DORs quickly identify which every document due across your facility and allows you to filter, review, and complete items before they become overdue.

When You’ll Use This Page

Use the Docs Due List when you need to:

  • Check which clinical documents are due today or upcoming

  • Review overdue documentation

  • Filter due items by therapist, discipline, document type, or patient

  • Confirm why a document is due based on visit count, scheduled sessions, or certification dates

  • Download a list of due documents for planning


Overview

The Docs Due List displays all documentation due for your facility. Each row shows the:

  • Patient name

  • Discipline

  • Document type

  • Assigned therapist

  • Due date

  • Status

  • Action

    • Create Document directly from the Doc Due List (If Applicable)

You can filter using the three-line sort icon next to each column header.

Document types that may appear include:

  • Daily SOAP Note

  • Evaluation

  • Progress Note

  • Recertification

  • Discharge

  • Addendum Note

  • Transitional Evaluation


Why Each Document Appears as Due

Documents Due (Progress Note, Recert, Discharge) will appear under the Therapist who completed the Evaluation.

IMPORTANT NOTE: Any therapist of that discipline can still complete the documentation even if assigned to another therapist.

Daily Notes

A Daily SOAP Note appears due when:

  • A therapy session was scheduled and no daily note or missed visit has been submitted

  • An Evaluation was completed (an eval session requires a billable daily note)

Important:
If an eval is completed but no daily note is submitted, the Daily Note will appear as due until completed.


Evaluations

Evaluations appear due when:

  • A new Episode of Care (EOC) is created and an eval has not yet been completed

  • A patient is added to the caseload via planning or schedule

Evaluations appear immediately upon admitting a patient to an EOC.


Progress Notes

Progress Notes follow visit-count rules and will appear due when either of the following is reached:

1. Visit Count Rule (Typical):

  • A Progress Note becomes due after a set number of treatment visits.
    (The exact number varies by payer or facility policy.)

2. Day-Based Rule:

  • A Progress Note may also become due after a certain number of days since the last Progress Note or Evaluation.

Once a Progress Note is due, it will show until completed.


Recertifications

Recerts appear due when:

  • The certification period is ending, and a new recertification is required

  • The therapist has reached the recertification visit count or time window

The due date is determined by the certification period in the Episode of Care.


Discharges

A Discharge shows in the Docs Due List when:

  • The anticipated discharge date has been reached

  • The end of the certification period has been reached without a new recert

  • A therapist sets an anticipated discharge date within Therapy Planning → three-dot menu

Important operational note:
For the discharge document to appear on the correct therapist’s schedule, the patient must have an assigned therapist for that discipline.

You can assign this by navigating to:
Patient Profile → Patient Details → Edit Details → Assigned Therapist → Submit


Addendum Notes

Addendum Notes appear when a previously submitted document requires an update or correction.


Transitional Evaluations

These appear due when a patient transitions between payer sources, programs, or clinical pathways and requires a new evaluation type.


How to Filter the Docs Due List

Use the filter icon next to each column header to filter by:

  • Therapist

  • Discipline

  • Document type

  • Patient

  • Due date

  • Status

You may also download the filtered list for planning or team huddles.


Common Questions

Why does a Progress Note show as due even though I just completed a discharge ?

Progress Notes follow visit-count and time-based rules, if a discharge is completed before the scheduled progress note the alert will show in the Docs Due dashboard. If you've already created a Progress note and the alert is still showing, please submit a ticket VIA the purple icon in the bottom left corner of the platform.

Why is a Daily Note showing as overdue when I did not treat the patient?

If a session was scheduled, you must submit either a Daily Note or a Missed Visit. The Docs Due List will continue to show it until one of these is completed. Contact your DOR to remove the patient from your schedule from the given day if you did not treat the patient. This should remove the alert from your dashboard.

Why is a Discharge showing for the wrong therapist?

Make sure the patient's Assigned Therapist is added in Patient Details. This determines whose Docs Due List and schedule the discharge appears on.


Troubleshooting

  • If a document appears due unexpectedly, check the Episode of Care to confirm certification dates and assigned therapist.

  • If Daily Notes appear unexpectedly, review the schedule for uncompleted or unmarked sessions.

  • If Progress Notes appear too soon, check whether multiple visits occurred since the last note.

  • If items seem missing, refresh your browser.
