Why Isn’t the Physician’s Name Appearing in the Evaluation Form?

The physician’s name on an Evaluation automatically pulls from the Primary Physician listed in the patient’s profile. If this field is missing or incorrect, the name will not populate in the Evaluation.

Overview

RESTORE-Insights pulls physician information directly from Patient Details to keep documentation accurate and consistent across the Episode of Care. The Evaluation form will only display the physician that is currently saved in the patient’s profile.

Updating the physician in Patient Details ensures all future documentation displays the correct provider.


How to Update the Physician in Patient Details

Follow these steps before starting an Evaluation:

Step 1 — Open Patient Details

  1. Go to Patients.

  2. Select the patient.

  3. Click Patient Details in the upper right corner.

Step 2 — Edit the Primary Physician

  1. Click Edit Details.

  2. Locate the Primary Physician field.

  3. Select the correct physician from the dropdown list.

  4. Click Save.


Verify the Physician in the Evaluation

  1. Open a new Evaluation for the patient.

  2. Confirm the physician’s name now appears automatically.

  3. Complete and submit documentation as usual.


Important Rules

  • The physician field cannot be edited directly inside the Evaluation. It must be updated in Patient Details.

  • Updates apply to new documentation only. Existing submitted documents will not change retroactively. You will need to add the physician to the patient details, un-sign, edit, and resubmit the documentation with the missing or incorrect physician name. (If this does not resolve the issue, please submit a ticket to our our support team to have this adjusted)

  • Only users with permission to edit patient demographics can update the physician name.


Common Questions

Q: I updated the physician, but the Evaluation still shows the old name. What should I do?
A: Make sure the Evaluation was opened after updating Patient Details. If the Evaluation was already submitted or started, you will need to add the physician to the patient details than un-sign, click edit, and resubmit the doc. (If this does not resolve the issue, please submit a ticket to our our support team to have this adjusted.)

Q: Why don’t I see the physician I need in the dropdown?
A: Physicians must be added to your facility’s provider list. Contact your regional or admin team to have them added.


Troubleshooting

  • If the physician name does not populate after saving changes, refresh the browser and try reopening the Evaluation.

  • If you cannot edit Patient Details, confirm your role permissions with your DOR or admin.

  • If the physician still does not appear, submit a support ticket through Intercom.
