1. Please edit the form and change the To email address to one of your personal email address like yahoo mail or gmail etc
2. Then, test the form submission and see if you get the email or not in your personal email address
3. If you got the email, then this is a mail server issue in your end.
Other possible reasons are
Form email is getting caught in your spam filter in email server or email client
Your mail server may not be relaying the email from our mail server (mail.revize.com)
Please contact your email admin to handle this issue
