Introduction for how to access and utilize Safeguard Bypass Planning

Overview

There are scenarios during steady state plant operation where a safeguard fails and needs unexpected maintenance, or needs to be temporarily bypassed for planned maintenance.

During these scenarios, it's typical in the Management of Change (MOC) process to understand the change and be aware of the exposure to risk during this temporary situation.

Safeguard Bypass Planning is a module which allows organizations to quickly visualize how a safeguard relates to its related potential consequences based on SME lead PHA Data, and understand through simulation how risk changes as Safeguards are bypassed. This helps a team understand their risk mitigation path forward and level of contingency required, and the importance of any other safeguards utilized for protection.

Accessing Safeguard Bypass Planning

Activating the features

Safeguard Bypass Planning features must be activated by your Risk Alive representative. These features can be activated for a single report to pilot its use in your organization, or we can turn these feature on for all reports. Just chat with us and we'll get you going!

Availability on specific reports

Safeguard Bypass Planning activation status, can be found by accessing "Live PHA Analytics" and selecting the Safeguard Bypass Planning tab.

Select "Review", highlighted below, to then see the projects which have this module activated.

Selecting a Safeguard for Visualization and Simulation

In order to start your Safeguard Bypass Planning process, you must first find the safeguard you are interested in.

You can Search and Filter for your safeguard of interest using description keywords for the safeguard, or by filtering for consequence category, severity, risk ranking and safeguard category.

Once you have identified your safeguard of interest, simply click on its description to be taken to that Safeguard's Bypass Planning visualization and simulation

Once a Safeguard is selected, the visual for Safeguard Bypass Planning will now be shown. All items highlighted below are selectable for additional information and to be able to simulate bypassing of Safeguards.

As per the visual below:

  • On the left, in blue, are all Causes in which the Safeguard selected is found on. You can hover over a Cause to see its full description. The Residual Cause Criticality of a Cause is shown below the Cause description.

  • To the right of the Causes, in green, are all other safeguards found on the scenarios related to the Safeguard selected. Only safeguards which have at least 1 occurrence of greater than 0 credit are shown. You can hover over a Safeguard to see its full description.

  • In the centre, in purple, is the description and icon of the Safeguard selected. You can hover over the center Safeguard to see its full description.

  • On the right, in grey, are all Consequences related to the Safeguard selected. Consequences are grouped by Consequence Category (e.g. Health and Safety) and Residual Risk Ranking (e.g. Very High). These groups show the number of scenarios within each pair and will change as simulations are performed.

  • In the top right, in orange, is a "View All Scenarios" link, which when selected will open a new tab and take a user to the LivePHA Project and filter for all scenarios containing the Safeguard selected.

You can select any of the highlighted boxes within the Safeguard Bypass Planning visual to see additional information. For example, in the Cause Drawer, you can see the full description of the cause, and the description of related consequences as well as Residual Risk Ranking.

Simulating Safeguard Bypassing

You can select any safeguard on the visual for Bypass Planning simulation. First select the safeguard you are interested in, and a Drawer will appear on the right.

At the top of the drawer will be a "Simulate Safeguard Bypass" option. Select this option if you want to simulate if a Safeguard were bypassed or no longer effective for protection. This selection will not impact any "source of truth" data in LivePHA; it is only a simulation.

Once a Safeguard is simulated for Bypass, you will notice a few changes on the visual and in the drawer

  • The Safeguard simulated (whether the center safeguard or non-center safeguards), will now appear Red

  • The Cause criticality will update to potentially be the same or higher due to there being less credited protection on the scenario

  • The Consequence Category/Risk Ranking Pairs will update:

    • The Number of Scenarios. Removing safeguards from protection will often increase scenario counts of higher risk, and reduce scenario counts of lower risk scenarios.

    • New Boxes may appear if new Risks arise. Removing safeguards from protection will often increase risk of scenarios.

    • The Drawer will show an update of risk ranking per consequence wherever a risk has changed.

  • All Safeguard simulations are sharable via the URL.

Restoring Safeguard Simulations

If you want to restore the bypass simulation of a Safeguard to its regular state, simply select the "Restore Safeguard" icon, viewable in the Drawer.
