What you need for your medical claim
No matter which provider you choose, make sure to collect the following documents to avoid delays in processing your claim.
1) Medical reports
A doctor’s report that includes:
Your symptoms
The date your symptoms started
The diagnosis
The treatment provided
You should also collect (if applicable):
Lab results
Scans or X-rays
An accident report (from police, ambulance services, or a tour guide)
A stamped prescription for medication
2) Detailed invoices
Itemised invoices clearly showing what you were charged
3) Proof of payment
Receipts or payment confirmations showing the invoices were paid
In some cases, additional documentation may be requested depending on the situation.
