Documents to collect for your claim
1) All reports for your injury or illness
Medical report from doctor which includes symptom(s), date of onset, diagnosis, treatment
Any other relevant medical reports such as lab work, scans, and x-rays
Accident report from police, ambulance, tour guide if applicable
Stamped prescription for medication if applicable
2) Detailed invoice(s) - showing what you paid for
3) Proof of payment - receipt(s) showing you paid
Additional information and documentary evidence may be required on a case-by-case basis.
