How to Complete the USPS Form 1583 for Mail Forwarding Authorization

Overview: What is the USPS Form 1583?

The USPS Form 1583, officially titled "Application for Delivery of Mail Through Agent," is a required document that grants permission for a mail receiving agent to accept mail items addressed to you. This form is required when setting up a new mailing address through Savvy Nomad's mail forwarding service. The 1583 only provides permission to SavvyNomad to receive your mail at your mailing address; completing the form does not reroute your mail in any way.

Completing the 1583 form correctly is one of the most important steps in the onboarding process, and it's also where many customers encounter issues. This comprehensive guide will walk you through every step to ensure your form is completed accurately the first time.

Understanding Photo ID vs. Address ID Requirements

Before you begin, it's important to understand that the 1583 form requires two separate forms of identification. You cannot use the same document for both your Photo ID and Address ID. This is a common mistake that causes delays in processing.

Recommended Approach: Use Your Passport

If you have a valid passport, we strongly recommend using it as your Photo ID for Box 8. Since you cannot use the same document as both your Photo ID and Address ID, using your passport as your Photo ID frees up your driver's license to be used as your address ID. (Despite contrary belief, your passport does not show an address and therefore cannot be used as an Address ID for the 1583 form.)

For Box 9 (Address ID), you can use any of the following documents:

What If Your Driver's License Has an Old Address?

Good news: If your driver's license shows an old address that is no longer current, that's perfectly acceptable. You're providing these documents to verify your identity using the IDs you currently have—the address doesn't need to match your current residence.

If you don't have a passport and are using your driver's license for Photo ID, you'll need an alternative for Address ID. Options include:

Note: If your documents are in a language other than English, please provide the original document along with an English translation. The translation does not need to be officially notarized—you can use any translation tool or service.

Important Restriction When Redoing the 1583

If you're completing this form again (for example, because a previous ID expired), there's one restriction: you cannot use an ID with your Savvy Nomad mailing address on it. You can use an address from anywhere else in the world, but not the Savvy Nomad address.

Step-by-Step: Completing the Form in the Customer Portal

Step 1: Select Your Address ID Type

When you reach the 1583 form step in the portal, you'll first be asked which type of Address ID you plan to use. This helps pre-fill the form correctly.

Select the appropriate option from the list. If you select "None of the above," you'll be provided with a link to generate a lease agreement to sign with a family or friend (anywhere in the world -- does not have to be within the US). This completed lease can then serve as your Address ID.

Enter the address exactly as it appears on your Address ID document. This is very important:


Step 4: Notarize Now

After entering your information, you'll see the USPS Form 1583 setup page with options for notarization. Select "Notarize Now."

You have two main options:

Option 1: Online Notarization with Proof.com

You can either start your notary session from within the portal or you can start it via email.

Starting the Session from within the portal

Click "Start Session" to begin. This button may take up to 30 seconds to appear after pressing the button that says "I am OK with the $25 fee." Important: You will be charged the $25 fee the moment you click this button, before your notary session begins.

After clicking, the system will generate your PDF and send it to Proof.com. This may take a few seconds.

Accessing Your Session via Email

You'll receive an email from Savvy Nomad containing a link to sign your document. Click "Sign document now" to proceed.


Creating Your Proof.com Account

When you click through to Proof, you'll need to log in or create an account. Note: This is a Proof.com account, not a Savvy Nomad account. The password you create here is for Proof.com only.

Reviewing Your Documents

Once logged in, you'll be able to review your form before signing. There are typically two documents to complete:

Make sure you sign everything! If you miss any signatures, you'll receive an email later asking you to complete the missing sections, which requires additional work to fix.

Multiple Businesses

If you have multiple businesses registered with Savvy Nomad, you'll see a separate 1583 form for each business. Each form will contain the same personal information but different business details.

Business vs. Personal Use

Pro tip: If you complete the form for business use, it covers both business AND personal mail. If you only complete it for personal use, you'll need to redo the entire process later if you add a business.

Filling in Box 8 (Photo ID)

In the Proof document editor, you'll need to fill in your Photo ID information in Box 8:

Common mistake: Some people enter their driver's license ID number in Box 8 while using their passport as their Photo ID. Make sure the ID number matches the document type you're using.

Filling in Box 9 (Address ID)

For Box 9, enter your Address ID information:

Date Format Tip

When entering dates, consider typing out the month name (e.g., "January 19, 2026") rather than using numeric format. This avoids confusion between American date format (MM/DD/YYYY) and international format (DD/MM/YYYY).

Identity Verification with Proof

After completing the form fields, Proof will ask for additional identity verification. This may include:

Option 2: In-Person Notarization

If you prefer to notarize in person, or if you don't have a Social Security Number (which Proof requires), you can complete the process manually.

Steps for In-Person Notarization:

Required Documents to Upload

When submitting your in-person notarized form, you must provide (1) Notarized 1583 Form (along with the Terms of Service), (2) Photo ID, and (3) Address ID.

Photo quality matters! Make sure your document photos are not blurry.

Processing Time

Note that in-person notarized documents require manual review and may take a few days to process. Online notarization through Proof is processed instantly.

Alternative: BlueNotary for Non-SSN Holders

If you don't have a Social Security Number, Proof.com cannot process your notarization. Instead, you can use BlueNotary:

What Happens After Completion

Once you successfully complete an online notary session, several things happen automatically.

First, your documents are generated and can be found in the documents tab. This includes your "Letter to the Tax Collector" to get your driver's license or state ID.


Second, the To-Do tab will have updated. Instead of guiding you through completing your 1583 Form, it will now be refreshed with new steps for you to complete.
