Medicaid Documentation in SLP Toolkit – HPC

If your district uses HPC as its Medicaid billing vendor, your district may have SLP Toolkit set up to collect documentation for Medicaid billing. In this setup, you’ll enter all required details in Toolkit, and your district plan manager will generate the billing report to share with HPC.

👉 Important: Not all districts using HPC have this integration set up. Some districts still require providers to document directly in HPC’s platform. Please confirm with your district Medicaid coordinator or plan manager to know which process applies to you.

If your district is using Toolkit for HPC billing, follow the documentation requirements below to make sure your sessions are eligible for Medicaid claims.

Documentation Requirements for HPC

Provider Information

  • Full Name: Use your legal name as it appears in your district employee records.

  • Employee ID: Enter your NPI number.

Student Information

  • Full Name: Use the student’s legal name.

  • Student ID: Enter the student’s school district ID number.

  • Date of Birth (DOB): Add the student’s date of birth.

  • DX Code (ICD-10): Select the ICD-10 diagnostic code that applies.

  • School District Name

Therapy Session Notes

  • Goal: Document progress toward the student’s IEP goal(s).

  • Notes: Include a description of the activity completed during the session.

Data for Billing Export

  • Service Type (CPT Code): Enter the correct procedure code for the service.

  • Place of Service: Enter where the session occurred.

When completing documentation, choose from the CPT codes listed in SLP Toolkit or enter additional ones as needed:

  • 92507 – Individual therapy session

  • 92508 – Group therapy session

  • 92521 – Fluency evaluation

  • 92522 – Speech production evaluation

  • 92523 – Speech & language evaluation

  • 92524 – Voice & resonance evaluation

  • 92526 – Swallowing treatment

  • 92605 – Eval for non-SG AAC

  • 92606 – Therapy for non-SG AAC

  • 92609 – Speech-generating AAC therapy

Manual entry (open field):

  • 92630 – Aud rehab, pre-lingual hearing loss

  • 92633 – Aud rehab, post-lingual hearing loss

  • 97129 – Cognitive therapy

  • 97533 – Sensory integrative techniques

  • 97535 – Self-care/home management training

  • 97550 – Caregiver training for patient functional performance

  • 92551 – Hearing screening

  • 92610 – Oral and pharyngeal swallowing eval

Manual entry (open field):

  • Agency

  • Home

  • Hospital

  • Worksite

Best Practices

✅ Always use full legal names (no nicknames).
✅ Ensure NPI, student ID, and ICD-10 codes are accurate.
✅ Double-check CPT codes and place of service.

What Happens Next

If your district has enabled Toolkit → HPC billing, your district plan manager will run and submit the HPC billing report. You do not need to take additional steps beyond documenting accurately in Toolkit.
