CMS Setup Page

Generally speaking you do not need to modify any of the settings on either the Setting or CMS Defaults tabs (CMS Setup page). This information is included for reference purposes only. To access the CMS Setup page you must be logged in using a System Administrator account.

Setting Tab

The CMS Setup > Settings tab is automatically populated with the settings you configured as part of the general configuration settings. A description of these fields is listed below:

Setting

Description

Printed CMS Version

Allows you to choose which version of the CMS 1500 form you’ll be using: 1500 (02-12) or 1500 (08-05). Note that 1500 (02-12) is the most current version.

Override CMS box 31

Box 31 is the Physician Signature and will show the name of Primary Worker. If you do not need this field on your CMS 1500 form click this option.

Override CMS Box 11c and 9d

This setting also affects Loop 2300 CLM06 of the EDI file. If you select this option CLM06 will show “N” indicating the signature is not on file.

CMS Default Tab

The CMS Setup > CMS Default tab includes configuration options to populate default values on the Service File and Service Event Cart line items for all clients in the database. Typically we do not recommend modifying these settings. A description of these fields is listed below:

Service File

The settings in this section will automatically populate fields on the Service File > CMS tab for all clients.

Use Service Start Date as First Symptom

To use the Service File Start Date to fill Box 14 (Date of Illness) on the CMS 1500 form.

Patient Status

To set a default marital status (Married, Single or Other) to populate Box 8 on CMS 1500 form.

Employment Status

To set a default employment status (Employed, FT student or PT student) to populate in Box 8 on the CMS 1500 form.

Referring Physician

To set a default to referring physician to populate Box 17 on the CMS 1500 form.

Line Item

The settings in this section will automatically populate fields in CMS box on Service Event > Cart tab for all clients.

Type of Service

To set a default Emergency Service indicator to populate Box 24c (EMG) on the CMS 1500 form.

Diagnosis Code (Pointer)

To show the relationship between service provided and diagnosis entered at Service File to populate Box 24e on the CMS 1500 form.

Note: The default Diagnosis Code Point is set to 1 indicating that the primary Diagnosis code should pull through to the EDI file. We do not recommend changing this from the default setting.

Modifier 1

For Modifiers 1 and 2 on the CMS 1500 form; to set a default modifier next to the CPT code in Box 24d and Loop 2400.

Modifier 2

Modifier 3 (EDI Only)

For Modifiers 3 and 4 (which appear in EDI only); to set a default modifier next to the CPT code in Loop 2400.

Modifier 4 (EDI Only)

Supplemental Information

To set default text in Box 24d on the CMS form (e.g. to explain unusual circumstances).
