The Score page displays a Score Summary graph that displays the progress of scored questions over time. There is also a score section for each time the Outcome Assessment is completed for a client.

It is possible to add notes to each score by clicking the Edit link located in the Scores section.

To display the questions related to the score click the Show/Hide Questions link. When completing multiple instances over time the change in score will be shown as well (overall change and the change for each question).
