How to use the forms section to create a rules exam/quiz that can be scored and verified.

Creating Rules Exam/Quiz

Navigate to the Forms area of the system.

In the top right of the screen click “Add Standard Form”.

Choose the recipients who will have access to the form:

  • Schools- At the school level in the association area

  • Officials- In officials accounts area

  • Coaches- Gives coaches the ability to fill out a form

  • Media- In the media accounts area

Choose “Rules Quiz” as form type, and give the form a specific name.

Choose the school year this form will apply to. NOTE: If opening up in the summer, more than likely you will want the form to apply to the following school year.

Position- Adjusts the ordering that the form will show up in the list for schools. Leaving at 0 is fine unless you want a specific order. The higher the number the more it moves to the bottom of the view.

Activity- If the rules meeting is specific to a sport/activity, select it from the dropdown.

Choose a Start time and date for the form to open. That will allow the form to automatically display for schools/officials and/or coaches at that specified time. (You can also open and close the form manually.)

Show to coach- Turn this to yes if you want this to display in the activity area the meeting form is designed for to allow coaches to click on the exam from the rosters, scores or schedules page. Otherwise the form will just display in the association area for the school.


Receipt Emails- if desired you can enter an email address to receive a notice every time the form is filled out.

Creating the custom form with quiz questions.

In the forms area, navigate to the custom forms area:

In the custom area select “Create Custom Form” from the top right portion of the screen.

Give the form a name. Ex: 2025 Basketball Rules Exam

Select the proper school year for the quiz.

Status should be set to Pending (This will be attached to the standard form which is described later).

Choose who should be able to view/complete the form (this should match the choices on the standard form).

Choose the activity the form applies to (again should match the quiz standard form).

Check the radio button that it is a quiz.

Click Save

Leave the default start and end times of 12/31/1969 (this leaves the form at its current setting) and mark the form as pending.

Adding Sections:

There is an option to add sections to your exam. This is not necessary but optional if you want to divide up the exam. To do so go to the tab that says Sections.

Then Click Add Form Section (Or the pencil icon by a section to edit ones that may already exist)

Give the section a name and make the section visible. The position number is the order in which the section will appear. The lower the number the closer to the top the of the screen the section will appear. Add any instructions that the section may require. Click save.

Adding Questions:

Click on the questions tab. Then click Add Question.

In the question area, type out the question. Mark that it is required and editable. Mark it as private. Positioning can be adjusted. The lower the number the question position the earlier it will appear. The Data Type should be String (text). Then put in the number of points the question is worth.

Help text allows for any clarifications that are needed for the questions. Rules reference will show up for questions that were missed after completion of the exam. Category needs to be Select (Dropdown). Then enter the answer choices.

Hit Save

After you save, you need to select which answer is correct, and save again.

Continue adding questions until all are added for the exam.

Connecting the custom form to the standard form

After you finish, go back to the standard form that was created for the exam/quiz.

In the area “Attached Custom Form”, choose the custom form you created for the quiz. Then either change the status of the standard form to Open, or select a Start Time and Date for the standard form to Auto Open. (Again the custom quiz form should remain set to pending).
