Which U.S. address do I use when filling out my registration and ballot request form?

The formal federal guidance is that you should register at the last address you lived at in the US, before moving abroad. It does not need to be an address you still have access to, and you will not need to receive any mail at it. This address is used to route your request to the right Local Election Official and ensure you receive a ballot that includes the right candidates on it.

I can't remember or find my old address where I was living in the U.S.

In order to register to vote, you will need to have an exact address. We've worked with some other voters who were able to track down their old address from tax records, insurance records, medical records, or even old photos or Google Maps.

Another idea is to reach out to the local election officials in the town or county where you used to live and ask if they have any other records they can help you track down. You can look up their contact info here: https://www.usvotefoundation.org/election-offices
