The follow & RSVP form

This is the form someone fills in when they follow or RSVP, and you control what it asks.

First name, last name and email are always collected. Everything else — resume, school, phone, graduation date and the rest — is yours to include, and each one can be required or optional. Three to five fields converts best.

Add up to three custom questions as short answer, single choice or multiple choice. Answers show on the person’s record and come through in exports.

Edit what people get after they follow or RSVP, including the subject line. Send yourself a test first.
