This article will talk you through how to make changes to the name of a medical questionnaire and also change the questions within the form.

To learn how to make changes to your medical questionniare, watch our short video. Alternatively, follow our simple steps below.
Step 1 :
From the main menu click the enter the settings area.
Step 2 :
Then once in settings, head to medical questionnaire section under the heading compliance.
Step 3 :
You will then be presented with your list of medical questionnaires. If you wish to change the name of the medical questionnaire, click the pencil icon.
Step 4 :
You will then just need to change the name of the form in the main box, which will update in the main list once saved.
Step 5 :
If you wish to change the main questions within the form. Select the add and edit items option.
Step 6 :
You will then be presented with the main text of the form, to edit a question you will need to hover over the side of the question and click the pencil icon.
Step 7 :
In the next section, you will be able to change the question being asked and the type of question as well if needed.
Step 8 :
If you wish to remove a question from the questionniare, you will need to hover over the question and choose the 'X' icon.
This will allow you to customise the form by removing any questions you do not require.
