This article will talk you through how to complete a medical questionnaire with your patients in clinic.
A medical questionnaire within Aesthetic Nurse Software allows you to obtain your patients previous medical history and any conditions they may currently have which may effect the treatment.
There are a number of branded medical questionnaires stored already in ANS, but we can always add any new questionnaires you may need specifically to your account.
To learn how to complete a medical questionnaire with your patient in clinic, follow our simple steps.
Step 1 :
The first thing you will need to do to fill out a medical questionnaire is book an appointment for the patient in the diary.

Step 2 :
Once booked you will need to click on this appointment in the diary to bring up the appointment summary and then choosing to view the patients record.
Step 3 :
Then when in the patients record, you will need to click on the appointment you wish to complete the medical questionnaire with the patient for.
Step 4 :
Once in the appointment you will be brought to our treatment documentation and your first section will be the medical questionnaire section. This section will pre populate with your questionniare set as default in your settings.
Step 5 :
If the questionniare set as default is not the correct questionnaire to be completed for this appointment. You are able to choose another questionnaire from the list provided.
If the questionniare you need is not within the active list, we may have this on file in our inactive list. To learn how to find our default list of medical questionniares, head to our specific guide on this feature.
Step 6 :
Once happy with the questionniare you can start completing this with your patient by going through each question and entering an answer into each box provided.
Step 7 :
Once filled out, you will just need to get the patient to sign the form when they are happy with their answers. You will then just need to select the save answers option to finish the form.
Step 8 :
Your medical questionniare hexgon will then turn green once the questionniare has been completed and the answer can be seen via the drop down provided.
Step 9 :
If you then need to complete another questionnaire you can choose another from the list and repeat the process for the second questionnaire.
This questionnaire will then be stored within this appointment, when the patient then comes to another appointment their questionniare answers will pull through to their future appointments, speeding up the process.
