How To Create A New Medical Questionnaire

Learn how to create a new medical questionnaire within ANS.

This article will talk you through how to create a new medical questionniare within Aesthetic Nurse Software. You may need to add a new medical questionnaire if you are unable to find one which fits your treatment or you have your own form which you would like on the software.

Medical Questionniare Set Up

First you will need to set up the main medical questionniare within Aesthetic Nurse Software.

Step 1 :

From the main menu, head to settings.

Step 2 :

Then when in settings, head to medical questions under the heading compliance.

Step 3 :

Once in this section, you will see a list of questionnaires already present. But to add a new one you will need to select the 'New Questionnaire' button.

Step 4 :

Then you will need to name the form how you would like it to be shown to your patients.

Step 5 :

Once happy with the name, select save details.

Step 6 :

This will then create a new form in the list. To start adding questions to your form select the 'Add and edit items' button.

Step 7 :

To start adding your questions, you will then need to select the 'New Question' button.


Question Set Up

Next you will be given an option of question types for the form. Each question type offers a different format.

Single Line Text Box

Your first option in the question box is a single line text box, this suits questions which require a short answer from your patients like gaining personal information such as First Name, Date of Birth or Telephone Number.

The optional tick box always a patient to miss the question rather than it being mandatory.

Once you click confirm, this question will be added to your form.


Text Area

Your second option is a large text box, this suits situations where you need a patient to list a number of things or give a detailed answer.

Once you click confirm, this question will be added to your form.


Yes Or No Question

The third option for your question type is the Yes or No option. This allows you to ask your patients simple questions where they can state yes or no.

Once you click to confirm, this question will be added to your form.

On some forms, you may have a yes or no style question followed by 'If yes please state' you should follow this question with a single line question to allow patients to give more details.


Multiple Choice

The next option is a multiple choice question, there are two ways to have the multiple choice the first is a drop down so the patient can select one answer.

To create a question like this you will name the question and then list all the options you would like your patients to choose from.

This will produce a question that allows the patient to select one answer.

However, you do also have the option to have a multiple choice question where the patient can choose multiple answers for this question.

You will just need to make sure you list your answer options and choose the ' can select multiple answers' tick box.

This will then produce the option so patients can select them as yes or no for each answer.


Line Break

Finally, you have a line break question. This isn't a question for your patients to answer but to format your forms.

You will need to enter a '-' in the question section, this will add a break between your questions to help make the questionnaire easier to read for your patients.

This in your forms template will produce a black line showing the break in questions.

The signature box at the bottom of the medical questionnaire with automatically populate when the form is generated for the patient.


Step 8 :

Once happy with the questions in your questionnaire, you can leave this section as your questionnaire automatically saves after each question is added.

To view your forms, you can head to the treatment documentation part of the software which will show you how the form looks for your patients. Your signature box for the questionniare will automatically populate at the bottom of the form.
