This article will talk you through how to deactivate your medical questionnaires from your forms list. Deactiving the forms will not delete them from your software but just move them off your active list meaning you can always reactivate them at anytime.
Aesthetic Nurse Software do also offer the facility to add your forms to your software for you. If you need help setting up your forms, just drop us an email.

Step 1 :
From the main menu, head to your settings area.
Step 2 :
Once in settings, head to the medical questions section under the heading compliance.
Step 3 :
From here you will be brough to your list of active medical questionnaires. To deactive a number of forms select the tick box next to your forms you wish to be removed.
Step 4 :
Once you have ticked the forms you wish to be removed, select the deactivate selected option and this will remove the forms from your list.
Step 5 :
Otherwise, if you only wish to deactivate one form you can click the eye icon to remove the single form.
Step 6 :
If you wish to retrieve the form, you will need to head into the non active questionnaires section.
