How To Create A New Consent Form

This article will talk you through how to create a new consent form with Aesthetic Nurse Software. You may need to add a new consent form if you are unable to find one which fits your treatment or you have your own form which you would like on the software.

To learn how to set up your new consent form, follow our simple steps below.

First you will need to set up the main consent form within Aesthetic Nurse Software.

Form the main menu, head to settings.

Then when in settings, head to consent forms under the heading compliance.

Once in this section, you will see a list of consent forms already present. But to add a new consent you will need to select the 'New Consent' button.

You will then be able to enter the name of the new consent form.

Then enter the main information of the form into the large text box.

Once happy with your form, hit save details.

You are able to add tick box options to your consent forms as well.

From the main form menu, click the add and edit item option.

You will then be brought to a page to add your tick box options to your consent form. You will need to select the new item button.

Then, you can enter what you would like the tick box to say.

Once happy with you tick box addition, you can select the add button.

This will then add the question to your form. To add another tick box option, you would just need to repeat the process again.

Once happy with the form, you are able to view the form by heading to the treatment documentation part of the software which will show you how the form looks for your patients.
