This article will talk you through how to complete a consent form within your Aesthetic Nurse Software.
A consent form is signed by patients to confirm their agreement to treatment and highlight the risks of the treatment. There are a number of branded consent forms which can be found in ANS already for practitioners.
To learn how to complete a consent form in clinic with your patients, follow our simple steps.
Step 1 :
The first thing you will need to do to fill out a consent form is book an appointment for the patient in the diary.

Step 2 :
Once booked you will need to click on this appointment in the diary to bring up the appointment summary and then choosing to view the patient record.
Step 3 :
Then when in the patients record, you will need to click on the appointment you wish to complete the consent form with the patient for.
Step 4 :
Once in the appointment you will be brought to your treatment documentation and you will need to select the consent form hexagon. Once selected, this section will pre populate with your consent form set as default in your settings.

Step 5 :
If the consent set as default is not the correct consent to be completed for this appointment. You are able to choose another questionniare from the list provided.
If the questionnaire you need is not within the active list, we may have this on file in our inactive list. To learn how to find our default list of consent forms, head to our specific guide on this feature.
Step 6 :
Once happy with the consent, you can read through the consent with the patient and once happy sign alongside the patient and hit save.
Step 7 :
Your consent form hexagon will then turn green once the conset has been completed and theanswers can be seen via the drop down provided.
Step 8 :
If you then need to complete another consent you can choose another from the list and repeat the process for the second consent.
