All medical and functional forms will contain a consent form, to be signed by the applicant, which acknowledges that the information provided is confidential and that they consent to participate in the medical. Generally, this will also be “witnessed” and signed by the consultant or admin officer.

If the worker does not wish to sign the consent, then the assessment cannot proceed.
