Carelever Forms and Consent
All forms processed through Carelever will include a consent section. This section must be signed by the applicant, acknowledging their understanding and agreement to the following:

They consent to participate in the assessment.
Additionally, the consent form must be witnessed and signed by the consultant or an admin officer.
Refusal to Sign
If an applicant chooses not to sign the consent form, the assessment cannot proceed. This is a non-negotiable policy designed to protect both the applicant and the integrity of the assessment process.
Accessing Consent Forms
For those completing assessments on paperwork, a copy of the KINNECT consent form can be downloaded from the applicant's appointment details in the Carelever Affiliate Portal. If your clinic does not have access to the portal, KINNECT will provide a copy of the consent form along with the necessary assessment paperwork.
Submitting Signed Consent Forms
Once the consent form is signed, please ensure it is sent back to KINNECT along with the assessment results. Email the signed consent form and results to pepresults@kinnect.com.au.
