Please ensure you fill out the ENTIRE document completely and legibly as this is a legal document
Please affix a pathology bar code label and hand write the security seal number on the printed chain of custody document.
Please obtain a copy of the FULLY COMPLETED CHAIN OF CUSTODY form with the bar code and security seal number on it and send a copy to pepresults@kinnect.com.au before you package it in the sealed pathology bag
