STEP 1:
Complete the drug and alcohol form as per normal process. When you indicate a drug class as non-negative, a chain of custody consent section will appear in the form, so you can obtain the consent from the candidate/worker to send the specimen for confirmation testing. Have the candidate/worker sign this consent.
STEP 2:
Return to the Affiliate App and click the Generate Chain of Custody button. This will ask you to confirm you have completed the necessary steps. If these steps have been completed, click the submit button to generate the chain of custody form.
STEP 3:
The Chain of Custody Form will open in another tab. Check the details are correct and all sections complete. If any sections are missing information, please return to the carelever DAS form to complete. Print off the Chain of Custody Form and then affix your barcode sticker and write in your security tag number on the printed document.
STEP 4:
Return to your DAS form and upload a photo of the barcode and security tag details on the printed form.
STEP 5:
Please obtain a copy of the FULLY COMPLETED CHAIN OF CUSTODY form with the bar code and security seal number on it and send a copy to resultsupdate@kinnect.com.au before you package it in the sealed pathology bag.
