Once you create a custom appointment form policy, you can tailor it to each account's needs: which fields appear, who can see and manage them, and which accounts the form applies to.
Important Note: The default "Standard Appointment Form" can't be edited. You can still assign accounts to it and set it as the default for new accounts.
Use the Configuration menu to navigate the Company menu.
From Company, click on Customize and then select Custom Forms and Fields.
Click 'Create Appointment Form Policy'
Another option is to Clone an existing appointment form and edit it.
Click on an existing Appointment Form to view details such as Overview, Field Configuration, Field Privacy, Policy Member and to preview the form itself.
Field Configuration tab
With a custom appointment form, you can:
remove optional standard fields
make optional standard fields required
add custom fields you created earlier, and make them required if needed
change where and to whom fields are shown
change who can manage the content of the fields
Fields that are required in all appointment policies display a red asterisk.
To add a field, select "Add Field To Form" in the top left corner.
A pop-up opens where you add the new field and choose its settings.
To change an existing field, select it from the list. To delete a field, select "Remove Field Configuration". If you don't see "Remove Field Configuration", the field is required and can't be removed.
Field Privacy tab
The Field Privacy tab controls when, during an appointment's lifecycle, interpreters can see specific information. It also controls what's included in the notification emails sent to interpreters.
Note: Access for requestors and LSC admins is set by their level of access, separate from these settings.
To change a field's privacy, select the field. You can choose whether to include the field in interpreter-facing emails, and use the multi-select box to choose the appointment statuses during which interpreters cannot view that field's content.
Click the 'Hide Interpreter Access' dropdown and select one or more options by holding down Ctrl and Enter to select/deselect options. Or use the select All checkbox option.
For example, a "Consumer Employer" field could be left out of emails while the confirmed interpreter still has access to it once the appointment reaches the "scheduled" status.
Policy Members tab
Once a policy is created, you add accounts so it takes effect.
Select "Add Accounts".
Select the accounts you want to move to this policy. To add every account, select the "All" checkbox.
Select "Add Chosen" to confirm.
Note: If an account already belongs to another policy, you'll get a confirmation prompt before it's moved. Select "Yes" to continue.
Form Preview tab
The Form Preview tab shows how the form looks and behaves for an LSC admin or requestor.
To change the field order, select "Reorder" in the top right corner from any tab.
The "Edit" button in the top right corner lets you change the name or description of the appointment form policy itself.
