Learn how to use Sitemate Links to receive incident and near miss reports

Single Submission Sitemate Links can provide anyone with access to submit a single form for one or more templates, via the Sitemate app without needing a Dashpivot account.

This works well for incident reporting, enabling anyone working on a site to report an incident or near miss electronically via the free Sitemate app, and submit the information directly into your Dashpivot account.

If your incident report template is setup as a workflow, you can set it up to notify someone at your company who has Dashpivot account once the report is submitted (eg. Safety Manager), using the workflow notifications feature.

You can even print out your Sitemate Link as a QR code poster and stick it on the walls around your site, and anyone who scans the poster with their smartphone will be able to submit an incident report.

You'll end up with a comprehensive register of all incidents and near misses in Dashpivot, with records automatically organised and easy to search/filter, so that details can be found quickly when needed.

In this scenario, none of the people submitting incident reports are paid Dashpivot users and do not have access to Dashpivot. Instead, they will use the free Sitemate app to submit reports for free.

How to create a Single Submission Sitemate Link for Incident and Near Miss Reports

  1. Go to the team folder where you want your reports to be submitted

  2. Go to the Templates section

  3. Go to the Links tab

  4. Make sure you are in the 'Single Submission' section (this should be the default). If you are in the 'Ongoing Submission' section and create a link, every person who submits a report will become a paid Sitemate Form User

  5. Click Create Link

  6. Give your link a name, and select your report Template

  7. Click create

Note: if you don't have an Incident and Near Miss Report template yet, there are several free templates available in the Public Template Library.

Once created, all you need to do is distribute the Sitemate Link to the relevant personnel, or stick up QR posters around your site(s) so anyone can fill out and submit an incident or near miss report if/when they occur.

How can workers submit an Incident Report?

There are 2 ways to distribute your newly created link:

  • Copy the link and send it as a URL eg. via email or text message.

  • Downloaded a QR code poster, which opens the link when scanned with a smartphone. Stick this on the wall of your site or office.

For this use case, the QR poster is a great option because it means anyone can scan the poster and submit an incident report. You wont need to know who this person is and send them a link directly - instead they can just scan the nearest QR poster and submit straight away.
