You can set how dates are displayed throughout your study, so every form reads the same way. It matters more than it sounds: a date written 03/04 means two different days depending on who is reading it.
Change the format

Go to Study settings.
Click the Options tab.
Click Edit.
Choose your date format.
Click Save.
đĄ Tip: agree the format with your sites before the study opens, and pick one where the month is unambiguous if your sites are in more than one country. It is much easier to settle this once than to work out later which reading a given date was entered in.
