Daybreak's Intake Questionnaire

In January 2025 Daybreak launched an exciting updated intake process in which a comprehensive intake questionnaire is now embedded into the onboarding packet for families so we can more quickly assess students needs and match them as soon as possible to a clinician. While Daybreak has traditionally used live intake appointments to assess students’ needs and to gather information that we use in the matching process, we’ve found that the intake appointment, can sometimes inadvertently extend time to care. For example - it may take a parent several days to schedule an intake appointment and then the child several more days to attend the appointment.

What is on Daybreak's Intake Questionnaire?

There are 10 questions on the questionnaire. In terms of the questions themselves, we are balancing a smooth/efficient process for families along with the need for therapists to have a broad picture of clients, allowing them an understanding of where they need to investigate further. Questions are pulled from the GAD2/PHQ2 (with one additional question) which are validated and reliable measures as indicators for mood, and basic biopsychosocial assessment questions that are typical within mental health treatment.

How long will the intake questionnaire take a parent to complete?

The entire onboarding packet should take a parent less than 10 minutes to complete.**Can I see what the experience looks like for a parent filling out the onboarding packet?*

What languages is the Intake Questionnaire available in?

The onboarding packet, including the Intake Questionnaire, is available in English and Spanish.

How can families get help completing the intake questionnaire if they have questions?

Will there still be any live intake sessions?

This intake questionnaire is also designed to identify if there is need for a more consultative assessment, and in those cases the student/family will still be matched with an intake specialist. We expect these live intake sessions to be necessary for ~5-10% of students referred.

Will I still be able to schedule live intake sessions for students at the time of referral?You will no longer see the option to schedule live intake sessions for students as part of the referral process.

Will I still be informed whether counseling is recommended or not after I refer a student?

Yes - if counseling is not recommended you will receive an email informing you and letting you know why. If counseling is recommended, you will receive an email once the student has been matched with a clinician containing the clinician's information and the weekly session date and time.
