How to create and customize review forms with different question types

Forms are the questionnaires that reviewers complete during a review cycle.

When you create a new cycle, you’ll be prompted to set up at least one form. Forms define what questions are asked, how they’re presented, and who sees them.

Conversation settings

Ask questions as they are' controls on how questions are shown to reviewers.

  • Enabled
    Questions appear exactly as written in the form.

  • Disabled
    Questions are reworded to feel more conversational, similar to a chat.
    The wording is automatically adapted for self-reviews and other review types.

This helps make reviews feel more natural and easier to complete.

Configuring forms

You can assign different questionnaires to different review types.

You can:

  • Use one form (questionnaire) for all review types

  • Create separate forms for managers, peers, self, and upward reviews (up to four total)

To add another questionnaire, click + Add form under the existing one.

The Allow to skip option on the form makes peer and upward reviews optional.
When enabled, reviewers can opt out by clicking Dismiss.

Welcome screen

The Welcome screen is the introduction reviewers see before starting the form.

You can:

  • Add context about the purpose of the review

  • Include instructions or expectations

  • Explain how the feedback will be used

Click Preview to see how the welcome screen will appear to reviewers.

Question options

Effy AI currently supports the following question types:

  • Rating scale
    Numeric or labeled scales (for example, 1–5 or Exceeds Expectations → Needs Improvement)

  • Open-ended text
    Free-form written responses

You can enable AI follow-up questions to collect deeper insights.

When enabled, Effy AI automatically asks a follow-up question based on the reviewer’s answer, tailored to their response.

When Ask questions as they are is enabled, you can customize separate wording for self-reviews, even when using the same questionnaire for multiple review types.

Notes on question types

  • Dropdown and table/KRA-style questions are not currently supported.
    For questions with many options, multiple choice is the closest alternative.

  • There is no combined “rating + comment” question type.
    To achieve the same effect, add an open-text question immediately after a rating question.

Editing forms after launch

Important: Forms cannot be edited after a cycle is sent.

If you need to make changes:

  • Create a new cycle with updated questions, or

  • Delete unsubmitted forms and resend the cycle with the corrected version

Scoring and reports

Effy AI calculates scores based on submitted answers.

  • Score configuration is available via the Set up report button.

  • Weighted scoring is not currently supported.
    All review types (self, peer, manager, upward) are weighted equally in aggregated scores and reports.

Workaround:
You can export raw data and apply custom weights in a spreadsheet.

Weighted scoring is on our product roadmap.

Final screen

The Final screen message is generated automatically and includes the name of the person who initiated the review cycle.
