1. Navigate to your External Form by clicking on the action menu

2. Click "Global Settings"

3. Click "External Forms"

4. Click on the external form you would like to update or create a new one

5. Head to the custom form questions section of your external form

6. Make sure to title your check box field with a descriptor of your image.

7. Click "Checkbox" in the drop down of field types

8. Click "Upload Image"

9. After uploading your image select "+ Add Field"

10. Click "View" at the top to preview your custom questions

11. Make sure your images are displaying and your form is ready to use.

12. Click the X to exit editing your external form

13. If a requester marked a box with an image you will see the Field Label with the word "yes".

In the example below the requester marked both Microphone and Podium on their external form.
