Create & Edit an Event Request Form

This guide provides instructions on how to create and edit an external event request form in Flowpath for your calendar events.

2. Click your user profile.

3. Click "Global Settings"

4. Click "Events"

Request Form

5. Click "Request form" or scroll to the section.

6. Determine if your form will allow submissions with conflicts or not and if a work order will be required or not for events.

7. Determine what locations are visible on the event request by quickly editing and selecting.

8. Same this with determining what work order categories are visible on the event request by quickly editing and selecting.

9. If layouts are enabled, you can select which layouts are visible on the events request form as well by quickly editing and selecting.

10. The welcome section is the top section of your external form. This section will only show if there is content added to it. To edit click the edit button.

11. Rich text editor is available to edit your Welcome Section. Once completed, click "Save."

12. You can add additional custom sections to the external form. The following questions in each section will be located after the location and time of event section on the form.

13. "No Section" is for all questions not grouped in a section you create. This section header will not appear on the form and will be listed as "Additional Questions." To edit the question(s) click on the edit. Click "Add Question" to add another question you do not want grouped.

Alert: Alert! One question will be required in the "No section" but you can have as many as you want.

14. When you edit the question a pop up will display that you need to fill out.

15. Select the field type you want for the question from the drop down list.

16. Add the question to a specific section if you have one created.

17. Determine if its a required field or not on your form.

18. Click "Update Field"

Adding Sections

19. Click "Add Section" to add a custom section.

20. Add your section title, description of the section using smart text editor, and select if it will be a required section on your form.

21. Click "Add"

22. Click "Add Question" to add a question(s) to the section you created.

23. Repeat steps 10-14

24. The difference is you select the section you created and want the question to be displayed on the form. You have edit capabilities on these to move as needed.

25. Click "Update Field"

Branching Logic

26. If you want to enable a branching logic for a section of questions to be displayed after a question is answered, you need to enable it within the question field labels.

27. Click "Select child section" and then select from a drop down of the sections you created for the form to be branching logic. The child section will only appear when the parent question is completed.

28. Click "Update Field"

29. You will have this icon when a question contains the branching logic.

30. Click "View" to preview your external request event form.

31. Click "Request Event" to see the external form.
