AUGUST 2025 PLAN UPDATE:
CCHP is exiting the Individual and Family Plan (IFP) market at the end of this year. We will continue to serve their Medicaid population (BadgerCarePlus), which makes up the vast majority of our membership.
New appointments for CCHP IFP members will not be scheduled after August 31, 2025.
Foodsmart will honor one final appointment, if already scheduled for the year of 2025.
Registered members will have access to the Foodsmart app and web platform to view information through the end of 2026.
NOTE: During initial CCHP visits, please ensure they are signed up for the Foodsmart app or web based product. This is critical for the success of this program and impacts patient volume - thanks!

Billing Codes:

Our contract with CCHP uses a different snt of CPT codes than the traditional MNT CPT codes of 97802 and 97803.
In a traditional setting, these codes are used on both the basis of complexity and on basis of time.
Because we want to maintain standardization across notes, we've received guidance from CCHP to go back to using time as the determinator of what codes to bill, instead of complexity, as we've been doing for the first half of the year.
As with any other plan, it is imperative that your note contains visit start and end times and total minutes spent providing MNT and that these numbers are reflected in both the units billed and the billing codes.
Every single note should contain accurate visit start and end times:
Visit Start Time: HH:MM, timezone
Visit End Time: HH:MM, timezone
Total minutes spent providing MNT: ___ minutes
For each of these codes, bill units per the 8-minute rule as usual (e.g. 1 unit = 8-22 min, 2 units = 23-37 min, 3 units = 38-52 min, 4 units = 53-67 min).
Physician Review for CCHP
For CCHP we are required to have our Medical Director review your charts before they can be sent for billing. We have built this workflow directly into FFD.
When you go to submit your chart for CCHP members, you will now see an option to “Send to physician for review” and will not be able to select “Finalize and submit to billing”.
Once you send this to our MD, he will either submit the claim on your behalf or provide feedback on something that needs to be fixed in your chart. If changes are needed, you will see a task show up with comments You will then be able to fix the chart and re-submit in the same way.
